Home Care support services
Fudul understands that living independently can be a challenge for many members of our community. We also know that times accepting help in your own home can be daunting and overwhelming, so we work hard to ensure that all of our clients and participants receive the best at-home care available. All of Fudul’s home care services are delivered with the client in mind, ensuring that our clients receive the kind of support that fosters independence and enhances their quality of life.
What are Home Care packages?
A Home Care Package (HCP) is a subsidy provided by the Australian Government to eligible senior Australians to engage them with a preferred approved provider, such as Fudul, who coordinates a package service to meet our client’s needs.
The Home Care Packages program helps senior Australians with complex care needs to live independently in their own homes. Individuals are assessed by a professional assessor using a nationally consistent assessment framework. During the assessment process, an individual’s priority for care is also determined.
The program has four levels of packages for different levels of care and support needs, with each level receiving a different amount of funding. The funds are paid directly to the provider of your choice who distributes those funds according to service needs.
Individuals approved for a Home Care Package can choose Fudul to deliver services in their home, or a home in a retirement village. Wherever they are, we will go to them.
What types of areas Fudul serve in
Our services take care of both the low and high care needs of clients living in the community.
Day Therapy
Personal health and well-being can be enhanced through daytime activities, specifically designed for older individuals, or individuals requiring specialised support for their health. Day therapy also helps you to engage socially and meet new people, which is also good for personal health and well-being.
Respite
Respite care provides full-time carers and clients with much-needed support, or even a break from their usual care schedule. When respite is needed, we offer both day and overnight respite care for clients. Our respite program can be provided in the home through personal carer placement. Community respite is also available. This is where clients participate in a community or social event, through a club group or centre as part of their respite experience.
Disability-Related Health Supports
The NDIS will fund specific disability-related health supports where the supports are a regular part of the participant’s daily life, and the need for the support results from the participant’s disability.
Disability Services
Our disability services offer daily care options needed for individuals to live independently and pave a promising future. This includes all of our services as listed and some may be funded through the National Disability Insurance Scheme (NDIS). Home Care Services
Personal & Domestic Assistance
We offer consumers assistance with personal care such as showering, dressing, personal hygiene, grooming, beauty as well as cleaning, laundry, and changing linen.
Shopping and Home Cooked Meals
We offer assistance with shopping and home-cooked meals that cater to client’s special diets for health, religious, cultural, or other reasons.
Gardening and Lawn Mowing
We offer assistance with gardening and lawn maintenance.
Transport & Social Support
We provide assistance with getting to and from doctor’s appointments, social activities, visiting family & friends, places of worship, banking, and shopping.
Hobbies
Carrying out activities like arts and craft, scrapbooking, sewing, knitting, card games, board games and listening to music etc
Group and centre-based activities
We provide assistance with getting clients together for group activities such as trips, cooking classes etc.
Occupational Therapy
Our Occupational Therapists will assess the provision of minor home modifications, equipment, and tools to assist consumers to keep living the way that they desire.
Technology Assistance
We can assist with emails, online banking & shopping, use of social media platforms and specialist services for eyesight and hearing.
Who can access the Home Care packages?
To be eligible for a home care package program, you need to be:
- 65 years or older, or
- 50 years or older if you identify as an Aboriginal and/or Torres Strait Islander, or
- 50 years or older and on a low income, homeless or at risk of homelessness
- An older person in need of services to help you stay in your home
- A younger person with dementia or other special care needs that are not met through other specialist services
- Approved through My Aged Care
- Home care services are available through government funding based on eligibility. For persons under the age of 65 who present with a significant disability can receive funding through the National Disability Insurance Scheme (NDIS) based on eligibility.
